Power of Attorney
Power of Attorney – There is a General Durable Power of Attorney and Health Care Power of Attorney. A General Durable Power of Attorney is a document that becomes effective only upon my disability such as; as a result of mental illness, mental deficiency, mental disorder, physical illness or disability, chronic use of drugs, chronic intoxication, confinement, detention by a foreign power, or other cause and must be accompanied by a physicians statement stating that the person is in fact disabled by such cause as listed above.
A Health Care Power of Attorney is a written statement of a person's health care and medical wishes including terminating care and ending life supports that are keeping a critically and terminally ill patient alive. Health care decisions include the power to consent, refuse consent or withdraw consent to any type of medical care, treatment, service or procedure.
